Covid-19 Employee Declaration
A form to be completed by an employee in advance of returning to work in the interests of safety of everyone on the premises. On review of this declaration, Management may request that the employee does not return to work immediately, and may contact them to discuss a suitable future return to work date. An employee may also be required to provide a fit to work cert in advance of your return to work.
Covid-19 Risk Assessment
This is a copy of a Risk Assessment for dealing with the current Covid-19 situation in the workplace. It is not likely to cover all scenarios and each employer should consider their own unique circumstances and amend this template accordingly.
